The Second International Symposium on Sport Concussion in Prague in 2004 led to the creation of a new Sport Concussion Assessment Tool (SCAT) for athlete self-evaluation and physician assessment of sports concussions.

The tool combines existing assessment tools, including the Standardized Assessment of Concussion, into a new standardized tool on a two-sided card and is a perfect summary to give to athletes and their parents after injury.

The form instructs coaches and parents on what questions to ask an athlete who may be suffering from a concussion. Athletes are asked to rate symptoms like nausea, headache, or sensitivity to light on a scale from 0 - 6, 6 being the most severe.

The form also demonstrates how a coach could test an athletes memory using word or number sequences. After the test a coach may should be able to score the athlete to determine further treatment.
